To find an approved doctor, lab or facility on the
health insurance:
Go to
www.emi-tpa.com
On the home page, left side-bar, click once on ECN Network
A drop down menu opens up. Click once on ECN Provider Directory
Move to the center of the page and choose your search
parameters.
To view claim processing / payments (EOBs):
Go to
www.webtpa.com
On the left side-bar choose member
This page requires a password and user ID for each adult person
covered, due to HIPAA. Select First time logon.
They will mail you your password.
Logon with your member ID and password.
Click Eligibility to see who’s covered, click claims to view
payments.
You can also find providers here, if you have a password and
member ID.
To find an approved dentist:
The Fire District recently changed to Guardian for dental
insurance.
Their website is
www.guardiananytime.com.
First sign up and note your login and password. Go to the tabs
across the top, scroll to the right and select Provider
Directories. Our Group policy is

To download a claim form and/or look up approved doctors
on the vision insurance:
Go to
www.visioncare.com
Select My Comp Benefits on the right side-bar, towards the
bottom.
If you are a first time user you have to register for a username
and password, click Register now, and follow directions.
Once you have a user name and password set up you can login.
Click Policy # for claim form. This claim form is to be used in
lieu of a vision benefit card, and authorizes benefits.
Click on
member name.
Your benefits will be displayed. Click Visionpass to download
claim form.
To download AFLAC insurance claim forms or Flexible
Spending reimbursement forms:
Go to www.aflac.com
In the upper left choose Claim Forms.
If you want disability or accident insurance claim forms, on the
map click Florida, then choose the form you want.
If you want a Flexible Spending reimbursement form, scroll to
the bottom of the page and under reimbursement forms choose Flex
