To find an approved doctor, lab, or facility on our health insurance:
Go to www.BCBSFL.com
Locate the quick links section on the left side of the screen and click on Find a Doctor or Hospital. Select your option from the list of providers, hospitals pharmacies, etc. Fill in one or more of the required name/plan fields (doctor’s name, plan name) Click the search button and you’re on your way!
REGISTER ONLINE WITH BCBS and check status of claims, view your benefits and other important information – Go to www.BCBSFL.com  create a User ID and PIN.

To find an approved dentist with you dental carrier:
Go to www.GuardianLife.com
REGISTER ONLINE WITH GUARDIAN and create a User ID and PIN. Use the tabs across the top to select a Provider Directory. Group Plan #00431843

To find an approved vision provider or request a vision voucher with your vision carrier:
Go to www.MyCompBenefits.com   REGISTER ONLINE WITH COMPBENEFITS and once you have a user name and password set up you can login.  Click Policy # for claim form.  This claim form is to be used in lieu of a vision benefit card, and authorizes benefits.  Click on member name. Your benefits will be displayed.  Click Visionpass to download claim form. Group Plan #VS 2801 

To find an approved vision provider without resistering:
Go to www.VisionCare.com click Find Vision Provider, 2nd pg. click Vision Care Plan, 3rd pg. enter member zip code (distance optional) and approved Providers should appear.

To download AFLAC insurance claim forms or Flexible Spending reimbursement forms:
Go to www.aflac.com
In the upper right choose Policy Holder.  Select the option needed and REGISTER ONLINE WITH AFLAC; create a User ID and PIN.  Use the tabs on the left side for your information selections.

To find an approved provider with your mental health carrier:
Go to www.MHNET.COM :     Select MEMBERS on tab to the left and follow the directions for provider and benefit information.  REGISTER ONLINE WITH MHNET; user name and password will be the Behavioral Health toll free number on the back of your BCBSFL ID card (without the dashes).
