To find an approved doctor, lab, or facility on our health insurance:
Go to www.BCBSFL.com
Locate the quick links section on the left side of the screen and click on Find a Doctor or Hospital. Select your option from the list of providers, hospitals pharmacies, etc. Fill in one or more of the required name/plan fields (doctor’s name, plan name) Click the search button and you’re on your way!
REGISTER ONLINE WITH BCBS and check status of claims, view your benefits and other important information – Go to www.BCBSFL.com  create a User ID and PIN.

To find an approved dentist with your dental carrier:
Go to www.DeltaDentalins.com
Register online, and once you have a user name and password set up you can login.  “Delta Dental PPO” is your dental network.  To find an approved dental provider without registering: Go to www.DeltaDentalins.com and on the right you will see "Find a Dentist". Select the “Delta Dental PPO” network and click search. Select your State and click submit. Then you can enter your zip code and other search criteria.

To find an vision provider with your vision carrier:
Go to www.EyeMedVisioncare.com   
REGISTER ONLINE WITH EYEMED and once you have a user name and password set up you can login.  “Access” is your vision network.  To find an approved vision provider without registering: Go to www.EyeMedVisioncare.com select the “Access” network and enter your zip code.  You will be asked to enter a text for security reasons.

To download AFLAC insurance claim forms or Flexible Spending reimbursement forms:
Go to www.aflac.com.
In the upper right choose Policy Holder.  Select the option needed and REGISTER ONLINE WITH AFLAC; create a User ID and PIN.  Use the tabs on the left side for your information selections.
